Antisocial Personality Disorder: prevalence and comorbidity in a sample of psychiatric inpatients

Objectives: Antisocial Personality Disorder (AsPD) often occurs in relation to a variety of mental disorders and this comorbidity seems to show more serious clinical implications. The aim of this study was to examine the prevalence of AsPD and its patterns of comorbidity with Axis I and other Axis II disorders in a sample of 361 psychiatric inpatients.

Method: to asses Antisocial Personality Disorder and other DSM-III-R Axis II disorders, the Italian version of the Structured Clinical Interview for DSM-III-R Personality Disorders (SCID-II) was used. To determine significant comorbidity between AsPD and Axis I-II disorders, the odds ratios were calculated.

Results: among our sample of psychiatric inpatients, AsPD showed a prevalence rate of 9,7%. Furthermore, AsPD was significantly associated with Psychoactive Substance Use Disorders, and frequently co-occurred with mood disorders and with schizophrenia and other psychotic disorders. The study of comorbidity with Axis II disorders showed a more frequent and significant association with Cluster B Personality Disorders. Interestingly, a significantly elevated odds ratio was found for comorbidity with Schizotypal Personality Disorder.

Conclusions: our study confirms the high prevalence of comorbidity of AsPD with both Axis I and Axis II Disorders. These results raise doubts about the nosological status of AsPD and the validity of its construct. AsPD might not represent a distinct pathological entity, but a spectrum of diseases reflecting heterogeneity with regard to etiology and response to treatment.
