In the last few years, many clinical and experimental studies investigated the role of different factors in the etiology and pathogenesis of the Borderline Personality Disorder (BPD). In the search of the etio-pathogenesis of BPD, the relative importance of biological, psychological and social factors was shown by different authors with divergent theoretical approach to the mental disease. Pharmacotherapy of BPD finds its theoretical and clinical framework in the psychobiological basis of the temperamental and spectrum dimensions, according to the Cloninger and Siever & Davis models. The targets of the pharmacological treatment are the psychopathological dimensions of the BPD, including symptoms of the behavioral/impulsive, perceptive/cognitive and affective spectra, rather than nosographic categories. The treatment of outpatients with BPD is difficult. Moreover, the occurrence of acutely stressful episodes as self-destructive behaviours, psychotic episodes, parasuicidal behavior and impulsive illicit drug intake often compromises therapeutics efforts. In this review update, the pharmacotherapy of borderline personality disorder is presented in the light of the hedonic homeostatic dysregulation (dyshedonia).
