In dealing with the violent patient in an emergency context, the legal aspects of involuntary treatment and professional responsibility of the psychiatrist are reviewed and the various procedures for patient restraint and involuntary treatment are compared. The medical management of violent outbursts aims at keeping the patient inside the health care system. In Italy, the lack of specific guidelines on how to face psychiatric emergencies leaves the psychiatrist in a controversial position, with an unclear profile of professional responsibility.
