The role of atypical antipsychotics in the primary or adjunctive treatment of bipolar disorder has not been completely explored. From the review of the current literature, a large amount of data is in favour of the use of these compounds, in particular olanzapine, in the treatment of mania or mixed-mania. Less information is available on the use of atypical antipsychotics in the treatment of bipolar depression and mixed states, as well as in the long term prevention of recurrences. However, all preliminary observations and the few controlled studies available seem to indicate an important role of these drugs in the treatment of bipolar disorder, especially in the most severe cases, characterised by psychotic features and refractoriness to the standard treatments.
