Atypical antipsychotics in psychobehavioural
syndromes due to neurological disorders
In the last years there has been an increasing interest in the pharmachological
treatment of behavioural problems and neuro-psychiatric symptomatology related
to several neurological illness, such as Alzheimer’s disease, cerebro-vascular
dementia and Parkinson’s disease. In the past, antipsychotics have been the
most largely utilized drugs in these conditions. The tolerability and safety
of atypical antipsychotics favoured their wide utilization in these situations.
In the present report we reviewed the available literature on the use of atypical
antipsychotics (Clozapine, Risperidone, Olanzapine, Quetiapine) in the neuro-psychiatric
syndromes related to different subtypes of dementia and Parkinson’s disease.
In dementia related psychoses the available literature indicated the efficacy
and safety of both Olanzapine and Risperidone. Olanzapine and risperidone resulted
also effective on the agitation associated with dementia, but risperidone was
more frequently associated with extrapiramidal side effects. Less information
is available on Quetiapina that, however, resulted promising especially in controlling
agitation and delirium. Orthostatic hypotension is the most frequent side effect
with this compound. Clozapine resulted particularly useful in treating psychoses
related to Parkinson’s disease.
