A case of cerebellar cognitive affective syndrome

Anatomical, physiological and functional neuroimaging studies suggest that cerebellar function can not be confined to the control of voluntary movement, but is also involved in the organisation of higher functions, such as cognitive and affective. So far, cerebellar anatomical and metabolic abnormalities have been shown in many psychiatric diseases such as schizophrenia, OCD and some mood disorders. Recently, a syndrome has been defined, which is characterised by alterations in the cerebellum-thalamus-cortical circuit, with specific neuropsychological features, such as negative symptoms with intellectual deficits. The hypothesis seems to be consistent with the clinical case of a patient showing affective flattening, social withdrawal and intellectual deficits, associated with vermal-cerebellar atrophy and altered cerebellar and subcortical perfusion, as detected by neuroimaging.
