A progressive change in the diagnostic criteria for mental disorders developed
in the last few years to define more precisely the various clinical syndromes
and their relationship to the different cultural and geographical contexts,
through a categorical approach. The authors have considered the area of schizophrenic
disorders from this standpoint.
Different diagnostic criteria for schizophrenia in two samples of patients hospitalized
in 1981 and 1991 have been compared and a new diagnosis has been formulated
using ICD 10 criteria.
A general diagnostic comordance for the schizophrenic disorders and substance
related disorders seem to indicate the difference between the categorical and
dimensional approaches in the diagnostic criteria.
In the authors' opinion these two different approaches have to be continuously
integrated in clinical work in order to evaluate and understand the individuality
of each patient. In this sense the psychoanalytic instrument may be important
and useful.
