Impulse dyscontrol can be considered one of the core symptoms of borderline personality disorder (BPD). In a dimensional model of mental disorders, impulsive behaviour seems to have an important role in the aetio-pathogenesis of a number of different psychiatric conditions. Impulse dysregulation and emotional instability are basic components of BPD but also of some affective disorders. Moreover, impulsiveness is important to explain a series of Axis I comorbid clinical conditions in patients with BPD, including mood disorders, substance abuse and eating disorders. In this update paper, the role of impulsiveness in borderline psychopathology was reviewed, with specific attention to the relative therapeutic perspectives. A structured therapeutic experience with the use of a new atypical antipsychotic drug was briefly presented.
