Obsessive compulsive disorder with poor insight: a review
Classical authors, like Lewis and Schneider, recognized that not all patients
with obsessive-compulsive disorder (OCD) had the same degree of belief that
their obsessive ideas are absurd or irrational and noticed that in some OCD
patients criticism and resistance towards symptoms decreased or disappeared.
These patients were variously classified as cases of obsessive psychosis, OCD
with overvalued ideas, and OCD with psychotic features. The DSM-IV has also
included in its official classification an OCD subtype with poor insight. However,
the clinical profile, hence, treatment indications of this subtype have been
investigated only in a few studies. This review describes and comments on studies
focused on this OCD subtype or on OCD subtypes with peculiar symptoms features,
obsessive-compulsive spectrum disorders and on response to drug treatment or
behavioral therapy. Furthermore, psychometric scales recently proposed to assess
poor insight are presented and their research perspectives are outlined.
