An uncertain diagnosis bears the quandary as to which treatments to use. Acute psychotic symptoms can pinpoint this difficulty, especially in individuals without currents mental illness. These symptoms might be ascribed to psychotic disorders, to affective disorders, i.e., bipolar disorder type I, or to substance abuse. The present case report will consider a typical clinical picture and will try to outline the pros and cons for drug treatment, in the acute and maintenance phases of type I Bipolar Disorder.
